You've noticed a widening par or thinning near your crown. Before you chalk it up to stress or genetics and move on, it's worth understanding what's actually happening beneath the surface, because for many women, this pattern of loss has a name, a hormonal driver, and nutrition connections worth talking to your provider about.
What It Is & Who It Typically Impacts
Female pattern hair loss (FPHL), also called androgenetic alopecia (AGA), is the leading cause of hair loss among women worldwide. It's a slow, progressive type of thinning driven by a combination of hormones, genetics, and how sensitive an individual's hair follicles are to those hormones.
FPHL can affect women at any age, but it shows up most often in two groups:
Women transitioning into menopause, due to the natural decline in estrogen during this phase
Pre-menopausal women with PCOS or other insulin-resistant conditions, where excess androgens play a bigger role
And it's more common than most people realize. Research shows 12% of women first develop clinically detectable FPHL by age 29, 25% by age 49, 41% by age 69, and more than 50% have some element of FPHL by age 79.
How It Works
Underneath the scalp, hair follicles sit within a rich network of arteries, veins, and nerves. Keeping this in mind helps explain why hormonal shifts and metabolic changes have such a direct line to hair growth.
In androgenetic alopecia, hair follicles are gradually miniaturized by a hormone called DHT (dihydrotestosterone), which is converted from testosterone. Over time, DHT causes each follicle to shrink. So as hair moves through its natural growth cycle — shedding and regrowing — each new strand comes in thinner and finer than the last.
This is exactly why FPHL is so easy to miss in its early stages. The change happens gradually, strand by strand, often over years, before it becomes visually obvious.
How It's Diagnosed & Treated
Because FPHL has a characteristic presentation, it can often be diagnosed through medical history and a physical exam by a board-certified dermatologist. Women who present with signs of hyperandrogenism (like excess facial/body hair or acne alongside hair thinning) should also be evaluated for underlying ovarian and adrenal disorders.
Medical treatment typically involves systemic and topical options that work by altering the production, transport, or metabolism of androgens like testosterone.
Common topical treatments:
Minoxidil: Originally developed as a vasodilator for hypertension, minoxidil is now used topically at 2% and 5% concentrations. It's one of the most well-established treatments for FPHL, working by stimulating the anagen (growth) phase of the hair cycle. One important note: it needs to be used indefinitely to maintain results — stopping typically means losing the progress made.
Melatonin: Produced in the brain's pineal gland, melatonin is being studied for its antioxidant and protective effects on the hair follicle.
Platelet-rich plasma (PRP): Uses concentrated platelets from the patient's own blood to stimulate follicle activity.
Microneedling: Creates controlled micro-injury to the scalp, which may enhance the effectiveness of topical treatments and stimulate growth factors.
Common systemic treatments include: oral minoxidil, finasteride, dutasteride & spironolactone - which all require prescription by a medical provider.

Nutritional & Hormonal Considerations
FPHL is so closely tied to hormones and metabolism, nutrition can play a meaningful supporting role, though it's rarely the whole picture.
Insulin and androgens are connected. Increased insulin is positively correlated with excess androgen production, which is part of why insulin-resistant conditions like PCOS carry a higher risk of FPHL.
Biotin isn't a cure-all. Despite its popularity in hair supplements, there's no research showing biotin supplementation helps hair growth unless someone has a diagnosed biotin deficiency, which is relatively rare.
Folate and B12 matter — if you're low. These nutrients support the metabolic activity happening inside the hair follicle, so correcting an actual insufficiency may be helpful, though supplementing without one likely won't move the needle.
Vitamin CÂ supports collagen production and iron absorption, both of which matter for follicle health.
Essential fatty acids may have inhibitory effects on 5-alpha reductase, the enzyme responsible for converting testosterone into DHT — a potentially useful mechanism for slowing follicle miniaturization.
Polyphenols are also being explored for their potential role in modulating hormone-driven hair loss.
The bottomline here: nutrition can support the hormonal and metabolic environment your hair follicles are living in, but it works best as a complement to medical treatment and an accurate diagnosis.

Androgenetic alopecia is common, progressive, and deeply tied to the hormonal changes happening in your body, whether that's the menopause transition or an underlying condition like PCOS. The earlier it's identified, the more treatment options (medical and nutritional) you have available to slow it down.
